Programs Evaluation
After participation in one of our programs, please take a couple minutes to download our Program Evaluation and fill it out. You can fax it (847.297.6179), mail it or drop it by either building. Evaluations allow our team the opportunity to keep what you liked and change what you did not. We thank you for your time.
Facilities
Rental RatesThe Golf Maine Park District facilities are available for varying fees based on room size, demand and residency.
For your convenience, rental applications can be filled out by downloading this form. Before filling out a rental application, please check to see if the date is available via telephone or in person. Once confirmed, you may mail, fax or bring the application to the office. Rental dates are not secured until the required deposit is paid. The balance is due 14 days prior to your function. Anyone making a reservation less than 14 days prior to selected date must either pay in cash or use a credit card.
The Cricket Pitch may be rented by the community or for league play. Any league play use requires a Park Use Permit prior to play. A Park Use Permit can be acquired by calling 847.297.3000 ext 22.

Administrative
If you are interested in sponsoring one of our youth sports teams please print and fill out this form. The cost to sponsor a team is $125. Please fill out the form and select the sport you would like to sponsor. Mail or drop off the application with payment to the park district.
If you are interested in renting any of our outdoor facilities including athletic fields, water spray park or picnic shelters please print and fill out this form. You may want to contact the park district ahead of time to see if the dates you are interested in are available. If you have trouble downloading this document, please call the Recreation Supervisor at 847.297.3000 ext 22.
If you are looking to receive financial aid for summer camp or another program please print out this form, fill it out and return to Feldman Park office. Staff will review and contact you with aid award if available.
If you are interested in an employment opportunity, please print application, fill out and return to the Feldman Park office in person or mail.
